ADVANCED OPERATOR/PASSENGER COURSE ($150.00 + $7.50 GST)

REFUND POLICY:
BEFORE THE PROGRAM OF STUDY STARTS:
(1). Written notice must be provided by a student to the institution, when the student withdraws from a course.
(2).If written notice of withdrawal is received by the institution less than seven calendar days after the contract is made, and before the start of a course, the institution may retain the lesser of 10% of the fees due under the contract or $150.00.
(3).If written notice of withdrawal is received by the institution less than thirty calendar days before the start of a program of study,the institution may retain 20% of the total fees due under the contract.
AFTER THE PROGRAM OF STUDY STARTS:
(1). If written notice of withdrawal is received by the institution, or a student is dismissed within 10% of the program of study's duration, the institution may retain 30% of the fees due under the contract.
(2).If written notice of withdrawal is received by the institution or a student is dissmissed, within 30% of the program of study's duration, the institution may retain 50% of the total fees due under the contract.
(3).If a student withdraws, or is dissmissed after 30% of the program of study's duration, no refund is required.
STUDENT DISSMISSAL POLICY:
(1).Absenteeism for longer than two consecutive days without informing the institution, and provision of a doctor's note, may result in a dismissal.
(2).Cheating and/or plagiarism.
(3). Behavior displaying insubordination, refusal to co-operate with the instructor's schedules.
(4).The use of alcohol or non-prescription drugs is prohibited while in attendance at the institution.
(5).Flagrant disrespect of the institution's rules of conduct and policies.
DISPUTE RESOLUTION POLICY:
(1).The institution shall designate a staff position to review complaints, if possible, someone not directly involved.
(2).The designated staff member shall request written submissions from all parties concerned with a dispute.
(3).The designated staff member shall review the submissions and conduct the necessary investigation.
(4).The designated staff member shall provide a written desision outlining the justification for the descision
to all parties involved in the dispute.
(5).All of the above conditions are to be completed within two weeks of filing of any dispute if at all possible.
I recognize and understand the risk involved in learning to ride, and riding a motorcycle, and I acknowledged that risk by signing the Comprehensive Rider Safety Training Course / Advanced Operator/Passenger Course Waiver Form. I understand that my performance will be evaluated and that I must satisfactorily complete each training phase before advancing to the next level. Should my instructor(s) determine that I am a danger to my classmates, or myself, I will not be allowed to continue the course. I understand the decision is for my safety and the safety of others and I will abide by the decision of my instructor. To receive the Pro-Line Motorcycle Training Completion Certificate, I must demonstrate
competent motorcycle operation by attaining acceptable scores on the skill evaluaton and knowledge test. Failure of the knowledge test or the on-cycle evaluation or not satifactorily completing a phase of the course is considered a failure for the entire course. Permission or recommendation to take part in the I.C.B.C. Motorcycle Skills Test may not be granted or advised by Pro-Line Motorcycle Training instructor(s).
There are no refunds for persons voluntarily withdrawing or being dismissed from the course. (Any refunds are at the discretion of Pro-Line Motorcycle Training management).
All students are required to be on time for each class session. All students are expected to participate fully in each and every exercise and to follow the directions of the instructor(s). Tardiness, absence from any part of the course, failure to follow the direction of the instructor or any irresponsible behavior on Pro-Line Motorcycle Training motorcycles/equipment that could or does result in any type of damage or accident may result in immediate termination in participation of the course without refund.
Pro-Line Motorcycle Training will supply all students with the use of a motorcycle, helmet and reflective safety vest during course training, M.S.T. testing and the I.C.B.C. Road Test (if necessary). Wireless radios will be supplied to students during the road ride section of training.
Pro-Line Motorcycle Training reserves the right to charge for any damage or loss of/to the motorcycle(s), equipment, property or training facility caused by the student.
If I elect to use my personal motorcycle for the Pro-Line Motorcycle Comprehensive Rider Safety Training Course, or the Advanced Operator/Passenger Course, I accept full responsibility for any damages incurred as a result of its use during my participation.
By signing this document, I hereby agree to follow the rules and regulations of the training course and that all information provided by me is true and correct.
